All copays, fees, and patient balances shall be collected at the time of service.

There is an additional $25.00 fee for completion of forms for use outside of patients medical record, including:

Life Insurance
Family Medical Leave
Short Term Disability
No Show Policy
To ensure appointment availability for all patients, individuals who accumulate three missed appointments without prior notice (“no-shows”) may be dismissed from the practice.
We kindly ask that if you are unable to attend your scheduled appointment, you notify our office as soon as possible so we can offer that time to another patient in need of care.
